目的探讨微创经皮椎弓根螺钉Sextant内固定术治疗高龄骨质疏松性脊柱骨折的疗效及对患者神经功能恢复的影响。方法选取88例高龄骨质疏松性骨折患者为研究对象,采用随机数字表法将其分为观察组与对照组,每组44例。比较两组手术相关指标、手术前后Cobb角、矢状位指数、术后ODI得分、Frankel改善级别。结果与对照组比较,观察组手术时间更短、切口更小、术中出血量更少、下床活动时间更早(P<0.05);两组术后Cobb角与矢状位指数均较术前显著改善(P<0.05),但组间比较差异无统计学意义(P>0.05);两组术后ODI评分均显著下降,且观察组下降幅度更大(P<0.0.5)。结论SPPSF可显著降低高龄骨质疏松性骨折患者的手术相关指标,保证凸畸形矫正效果,有效缓解神经损伤,提高患者生活质量,值得临床推广。
ObjectiveTo study the efficacy of minimally invasive percutaneous pedicle screw fixation using Sextant system in the treatment of senile osteoporotic spinal fractures and its effect on neurological function recovery. MethodsEighty-eight senile patients with osteoporotic fractures were selected and were divided into observation group (n=44) and control group (n=44) using random number table. The surgery-related indicators, the preoperative and postoperative Cobb angle and sagittal index, postoperative ODI score and grade of Frankel improvement were compared between the two groups. ResultsCompared with the control group, the operation duration, the incision length, intraoperative blood loss and duration for ambulation were less or shorter in the observation group (P<0.05). The postoperative Cobb angles and sagittal indices were significantly improved in both groups, but there were no significant differences between the two groups (P>0.05). The ODI scores in both groups decreased significantly after operation, and the descent range of the score was greater in observation group compared to the control group(P<0.05). ConclusionFor the elderly patients with osteoporotic fractures, percutaneous pedicle screw fixation using Sextant system can significantly improve the surgery-related indicators, can ensure the correction efficacy of convex deformity, can effectively relieve the nerve damage and improve the quality of life. This approach is worthy of clinical promotion.
